6/11/2023 EnchantaVaughn Approved 1.Behavior: QMHPA arrived to Kaila home to conduct an individual session. Kaila appeared to be upset evident by her facial expressions such as frown on the face. QMHPA inquired about her day. Kaila verbally discussed to QMHPA that she was frustrated and irritated. During this session QMHPA will discuss Kaila anger issues on effective communication and appropriately controlling anger brought on by daily stressors. Kaila did not present any symptoms of S/I or H/I at the time of the session. 2.Behavior: During this second portion of the session, Kaila was observed presenting hopeful behavior when wanting to get control over her emotions and anger. 3.Behavior: During the third portion of the session, Kaila positive about learning healthier ways to manage her depression and anger. 1.Intervention: QMHPA provided Kaila with tips to assist her with identifying how body language plays a major role in how someone responds to her. QMHPA stated that her demeanor may cause others to take offense and negatively react to her actions and body language. QMHPA role played with Kaila to assist her with coping with her negative attitude and anger. QMHPA implemented ways in which she could control her anger with healthier outlets such as recognizing and verbalizing the how irrational thoughts contribute to the emergence of anger. QMHPA also encouraged her to replace negative feelings with those that are positive using guided imagery. QMHPA discussed the importance of effectively communicating with others especially her son as she tends to argue with on a daily and consistent basis. QMHPA stated that communication is the key when trying to get your point across however listening plays a role as well because if you talk while others are trying to speak you are breaking the barriers to effectively communicating. 2.Intervention: QMHPA implemented role play scenarios to aid consumer in effective communicating. QMHPA accompanied Kaila out into the community to gather some items for herself. QMHPA encouraged Kaila to only purchase those items that were necessary such as hygiene products, shoes, under garments and etc. QMHPA processed with Kaila on how she just received her benefits for the month; therefore, she needed to ensure that she was budgeting her money. As QMHPA and Kaila were walking down the aisle, QMHPA role-played with Kaila on budgeting. QMHPA educated Kaila on how she needed to purchase items that were less inexpensive so that she would have enough funds to last her the remainder of the month. Once returning back to the home, QMHPA reviewed and implemented learned techniques to assist Kaila with controlling and managing her anger and depression. 3.Intervention: QMHPA implemented deep breathing, temple massaging, counting and taking a time out to engage in leisure activities. QMHPA provided Kaila with tips and pamphlets on how to control her anger/depression. QMHPA processed tips and information with Kaila and encouraged her to be mindful of her actions on a daily basis especially in the presence of her daughter. QMHPA identified ways for Kaila to relieve stress which included her being able to appropriately control her anger through healthier outlets. QMHPA encouraged her to not let things and the situation with she and her family take her out of her element as it would affect her in the long run. QMHPA provided Kaila with support through body tone, listening, paraphrasing and summarizing. QMHPA taught Kaila to ways to relieve stress and asked her to write down her feelings as a means of relieving stress and getting her feelings out without exploding and presenting inappropriate actions. 1.Response: Kaila was open to the interventions that was provided by QMHPA. Kaila stated that she is in knowledge of techniques and strategies provided; however, it is all out the window when she is put in the situation. Kaila stated that she canât help her body language as she stated that it was a part of her and she didnât know how to change it. She also stated that she does; however, understand how her how it can come off as being negative because of her facial expressions and demeanor. 2.Response: Kaila reports being as though she has a QMHPA who motivates her and assists her with areas in her life she doesnât understand or know how to control (i.e., anger, depression) she feels as though she is going to make progress in the future. 3. Response: Kaila was able to discuss with QMHPA on ways to implement calming techniques to assist her with managing her depression. Kaila discussed with QMHPA on how she has to learn how to manage her anger when she is in the midst of her children. She reports she is wanting to learn how to be a better parent because she doesnât want her children to have the same behaviors when they get older. She wants to be able to manage her behavior at all times.
